As per the attached image, header levels 1 and 2 are lining up very nicely in the table of contents.
However, level 3 seems to insert a long row of dots between the numbering and the text. This causes these items to not line up correctly on the right, and makes the appearance quite bad.
What is causing this and how can it be fixed?

Thank you for sharing the file. This is because it uses standard tabs that are not wide enough for numbering. You can change the styles to use larger tabs or regenerate the table of contents without tabs. To do this:
Click the ribbon command References | Table of Contents group | Click Generate > uncheck “Keep tabs after numbering.” It should look better this way.
